The health department said it has received a series of complaints from people who had recently eaten at Bonicki's Restaurant and Sports Bistro, 1891 E. Apple Ave. The cause of the illnesses has not yet been determined.

Health officials are asking anyone who has become ill after eating at Bonicki's to fill out an online questionnaire. CLICK HERE for the questionnaire.

If you would like to report an illness and do not have a connection to the Internet, you can call 231-724-1202.

Bonicki's remains open and workers told WZZM-TV that they are fully cooperating with the health department.

This is the third illness outbreak at a West Michigan restaurant in the past few weeks. Last week, more than 300 people fell ill following a norovirus outbreak at the Wild Chef Japanese Steakhouse in Holland Township. Last month, 60 people contracted norovirus from the Beltline Bar in Grand Rapids.

If they were cooperating with the health department why did so many people get sick? Either they just started after their nasty practices caught up to them or the standards and inspections set forth by the health department are inadequate.
